    I can’t imagine a healthcare provider doing this but obviously it happens. They certainly may think it, or even say it to peers, which would get them some side eye, but at worst they’ll put “unreliable historian” or “noncompliant with instructions” in the documentation. Especially documentation the patient could see. How stupid and unprofessional.

    We should all get our own Medical Consult letters, I have discovered so many errors, whether on purpose or mixed up with another patient. If you never get to read those documents how do you know.

    I always go begging and get a hard time, it takes months to get the records.

    The paper records for my wife’s previous doctor were given to me because she is switching doctors. She was reading reviews from other doctors and specialists who described her as “pleasant” and “very pleasant,” which made her wonder if there was some kind of code being used.

    We went together to a fertility clinic where it said “I met with very pleasant M and her morbidly obese husband”.

    Okay, thanks.
