Medical malpractice insurance is so costly now that virtually no doctors can go into private practice straight from med school or right after finishing their residencies. This is presumably due to a very high rate of malpractice lawsuits being lodged by patients or their representatives, cases that are legitimate and awards having to be paid out to them by insurers.
So many serious injuries and even deaths occurring here and unfortunately plenty of it is due to incompetence, by doctors, other healthcare workers or a broken system.

One would think that taking out the wrong bit of the brain is a disqualifier for a neurosurgeon

The difference between God and a surgeon is that God knows he is not a surgeon, and this man makes me think of an old joke about that.
